If you are seeking a dynamic role in public service, join Chesapeake Integrated Behavioral Services Mental Health Case Management team and help connect individuals to the services and supports that promote recovery, resilience, and long-term stability!

This position is responsible for assessment, service planning, care coordination and ongoing monitoring to ensure appropriate utilization of services and continuity of care.

Typical tasks include:
  • Consumer assessment, treatment planning and monitoring
  • Community outreach to assist consumer in maintaining treatment compliance
  • Documentation of all services in electronic medical record
  • Quarterly reviews of consumer progress and of active service plans in compliance with Medicaid and DBHDS requirements
  • Coordination of consumer services with services providers, community partners and familial supports on behalf of consumer
  • Assist with discharge planning for consumers that have been hospitalized, including linkage to community resources
