Floyd Valley Healthcare in Le Mars, IA has a full-time position (40hours per week) in our Home Health/Community Health Department Manager (RN required).

Responsibilities:

• Act as the liaison between the hospital’s Administration, the Board of Trustees, the staff and the community.
• Responsible for the care provided to our home health patients and for the affairs of the Community Health Department.
• Provide leadership to skilled services in Medicare Certified Home Health agency including nursing, home care aides, and therapy services.
• Ensuring compliance with Medicare Conditions of Participation.
• Grant management of public health services for Plymouth County.
• Continue evaluation of community health needs and development of health improvement plans.
• Participate in community and professional organizations to enhance home health and public health programs.

Requirements:

• Previous management experience is a plus.
• Licensed as a registered nurse (RN), in Iowa and/or meets the requirements of multi-state licensure required.
• Valid driver’s license and proof of insurance.
