ABOUT THE POSITION

There is one (1) fixed-term Social Service Worker II, Bilingual (Spanish and English) vacancy through 6/30/2026, within Behavioral Health and Recovery Services in the HOPE program. The individual will be performing supportive case management services (such as accessing food resources, eligibility to entitlements, re-certifications, etc.) to Older Adults (60+) experiencing mental illness who may also be experiencing co-occurring conditions such as substance use or medical complexities. The individual will be working as part of a multi-disciplinary team to provide integrated care to meet the multiple needs of the clients. This individual will be performing complex care coordination for Medi-Cal beneficiaries across multiple systems of care (MH, SUD, Dental, Medical, Housing, Legal, Occupational, etc.). The individual will also be conducting outreach and engagement with the LatinX community along with working beside the Senior Peer counseling supervisor in re-establishing the ACASA program (a Senior Peer Counseling program providing support to monolingual Spanish speaking older adults). They will also be conducting needs assessments, develop care plans, and coordinate the care of the older adult clients with various partners both internal and external.
 
The fixed term position will be initially funded up to June 30, 2026. Please note, this position may end prior to June 30, 2026, if funding ceases or the work is complete. In addition, these positions may continue after June 30, 2026, if funding is renewed or extended. Per PMR 48.2, fixed term employees are not subject to Reduction in Force rights at the completion of the term. 

Bilingual Testing/Certification: Successful candidates must demonstrate proficiency in both Spanish and English prior to appointment and receiving differential pay. If the selected candidate has not taken the test, a bilingual proficiency exam will be administered to ensure the candidate possesses the appropriate skill level to meet the requirements.


ABOUT YOU

Our Highly Qualified Candidate: 

Utilizing the Assertive Community Treatment (ACT) model, the HOPE program provides intensive psychiatric case management services that are accessible 24/7 to older adults age 60 and over. Services are provided to individuals experiencing serious mental illness, co-occurring substance use and/or medically complex issues. Individuals may be experiencing episodes of homelessness; frequent hospitalization/crisis service utilization; or justice-involvement. In addition, the team provides on-going assessment, treatment planning, recovery-based services, and uses a "whatever it takes" approach to meet program goals.
 
Under the direct supervision of the HOPE Unit Supervisor, this ideal candidate will be engaging in a multi-faceted role which includes providing case management, securing and coordinating community resources, coordination of specialty/complex medical care and follow ups, complex housing coordination and data entry relating to Medi-Cal billing. The candidate will be able to work collaboratively and effectively within a multidisciplinary team. The candidate will possess strong written and verbal communication skills and have experience using electronic records for documentation. The candidate will also act as an advocate for the client along with empowering the client towards their goals of recovery. The ideal candidate will provide in Spanish, excellent outreach and rapport building skills with individuals and families and values providing high quality, efficient, comprehensive care coordination services within a linguistically and culturally competent framework.

Qualifications

Knowledge of:

  • Principles, practices and techniques of social work.
  • The role and function of a public social services agency.
  • Principles and techniques of interviewing.
  • Methods of social work record keeping, including computer applications related to the work.
  • Community organizations and resources and the social problems calling for the use of public and private resources.
  • Social research methods.
  • Basic principles of public assistance policies and programs.
  • Laws, rules, and regulations governing the operation of a social service agency.
  • Goals of and current trends toward client rehabilitation.
  • If a position is in the HIV Program, the medical, psycho-social and economic components of the various stages of HIV.
Ability to:
  • Analyze client situations and social services needs.
  • Develop, prepare and implement effective case work plans.
  • Apply applicable laws, rules, and regulations to social service operations.
  • Interpret social service programs to applicants, recipients and others.
  • Use appropriate community resources and services effectively.
  • Utilize supervision and direction effectively.
  • Direct the work of others on a project basis.
  • Prepare clear, concise and accurate case records, reports, correspondence and other written materials.
  • Maintain accurate records and files.
  • Establish and maintain effective working relationships with those contacted in the course of the work.
Minimum Qualifications
Either possession of a Master's Degree in Social Work or a Master's degree in a two year counseling program by time of appointment, or possession of a Bachelor's Degree or equivalent with coursework related to the field and four years of journey level social casework experience of which two years were in adult social services at a level comparable to the County's class of Social Service Worker I or Social Service Worker I - Bilingual. Demonstrated oral and written proficiency in the designated second language is required.

Candidates selected are subject to fingerprinting by the Sheriff's Department and must pass a Department of Justice LiveScan background check prior to appointment.

Disaster Service Workers: All County of Marin employees are designated Disaster Service Workers through state and local law (California Government Code Section 3100-3109). Employment with the County requires the affirmation of a loyalty oath to this effect. Employees are required to complete all Disaster Service Worker-related training as assigned, and to return to work as ordered in the event of an emergency.
